Boarding Forms
Planning to board your pet with us? We require all boarding guests to be current on vaccinations before their stay. To make check-in faster, please download and complete the appropriate boarding admission form above ahead of time. Frequently Asked Questions — Online Forms
Can I fill out my forms before my appointment?
Yes — we encourage it. Completing your forms at home means a faster, smoother check-in experience. You can email completed forms to pethealth@arrowheadvetclinic.com or simply bring them with you.
What forms do I need as a new client?
New clients should complete the New Client Registration Form. If your visit includes a wellness exam, you may also want to complete the Wellness Healthcare Questionnaire. Both are available for download on this page.
Do I need to complete a boarding form before dropping off my pet?
Yes. We ask that all pet owners complete a Boarding Admission Form before their pet’s stay. Use the Clients form if you are an existing patient, or the Non-Clients form if your pet is boarding with us for the first time. Learn more about our requirements on the Boarding page.
What format are the forms in?
All forms are PDF documents. You’ll need Adobe Reader or a compatible PDF viewer to open and fill them out. If you don’t have Adobe Reader installed, you can download it free at adobe.com.
Can I email my completed forms before my visit?
Yes. Once completed, email your forms to pethealth@arrowheadvetclinic.com. You’re also welcome to bring printed copies with you to your appointment.
